Managing Stomal Bleeding: Seeking Advice and Shared Experiences

This topic is about managing bleeding from a stoma, which has been a concern for someone who has had a urostomy since 1994. They have been using a Convatec wafer and pouch, but in the last couple of years, they've noticed intermittent bleeding from the stoma. Here's what they've tried so far:

- Applying direct pressure to stop the bleeding.
- Using silver nitrate sticks to cauterize the bleeding point, as advised by their urologist.
- Visiting the emergency room three times this year, where during one visit, a doctor sutured a tear on the stoma, which stopped the bleeding for about six weeks.

They are reaching out to see if others have experienced similar issues and are looking for solutions that don't involve surgery. Here are some helpful suggestions:

1. Seek specialized assessment:
- Schedule an appointment with an ostomy (WOC) nurse to examine the stoma and check the fit of the appliance.
- Consider consulting a gastroenterologist or a colorectal/urostomy surgeon, as minor revision surgery might be an option.
- If the issue continues, getting a second or even third medical opinion could be beneficial.

2. Review appliance fit and handling:
- Ensure the wafer opening or its edges are not rubbing against the stoma, as this can cause bleeding. The hole should be cut or molded precisely to fit.
- Check if a belt or other support is pulling the wafer upward and causing chafing on the stoma.

3. Consider switching products:
- Try different wafers or pouch systems to see if a better fit reduces trauma.
- Convatec Moldable Technology wafers are recommended for their pliable material, which can reduce edge irritation. You can request free samples from Convatec before making a purchase.

4. Ongoing monitoring:
- Keep an eye on the area that was previously sutured for any signs of recurrent tearing or infection.
- Document any bleeding episodes to provide detailed information during future medical visits.
