Need tips for adhering appliance on scar tissue?

This topic is about finding effective ways to adhere an ostomy appliance to slick scar tissue around a revised stoma. After undergoing stoma-revision surgery, some people find that the scar tissue that forms can make it difficult for the wafer to stick securely. Here are some practical tips and product suggestions to help improve adhesion:

- Build a multi-layer seal:
1. Use a Hollister Adapt CeraRing around the stoma.
2. Add a belt and large half-moon barrier supports for extra security.
3. Opt for a flange with a cloth-like outer adhesive instead of a plastic-like one.

- Substitute products for the discontinued Hollister 7730 spray:
1. Nu-Hope Adhesive Cement (be cautious if you have sensitive skin).
2. Torbot “Product 375” liquid adhesive.
3. Urocare #500503 Uro-Bond silicone adhesive.
4. Manfred Sauer UK Ltd latex-based skin adhesives, available in various formulas.
5. Prosthetic Solutions Ltd Zeflosil Silicone Adhesive.
6. Salts Healthcare Latex Adhesive Solution.
7. Technovent Ltd G626 Probond Secure Adhesive.
8. Z-type hydrocolloid alternatives and seals like Ainscorp Aloe Rings and Dansac TRE Seal.
9. Search online for “7730 medical adhesive” for more options.

- Use a skin prep/barrier and paste combination for uneven or slick skin:
1. Apply 3M Cavilon No Sting Barrier Film on all areas where the wafer will contact the skin.
2. Use Coloplast Brava Tube Paste around the stoma before applying the wafer.

- Appliance choice and warming:
1. The Coloplast SenSura Mio Convex 2-piece Click wafer molds better if warmed with your hands or body heat before application.

- Post-application heating:
1. After applying the pouching system, gently warm the flange with a hairdryer. This gentle heat helps the adhesive conform and bond to uneven or scarred skin.

- General tip: Experiment with different combinations of seals, sprays/adhesives, barrier films, and supports, as scarred surfaces can vary widely in texture and moisture.
