Tips for Easier Ileostomy Appliance Changes and Skin Care

This topic is about making the process of changing an ileostomy appliance easier and taking care of the skin around it. An ileostomy can sometimes lead to leaks, which can be uncomfortable and irritating, especially when the output is pure bile. Here are some helpful tips and insights to make the process smoother and more comfortable:

1. Consider switching products:
- Some users find that Hollister Adapt Barrier Rings are easier to remove than Eakin Seals.
- Using a convex flange, with or without a seal, can help achieve a tighter fit and reduce leaks.

2. Cleaning and preparation techniques:
- Use only an adhesive-remover wipe and avoid skin-prep or barrier sprays, as they might interfere with the seal.
- Wash the skin around the stoma with a mild, oil-free, and perfume-free soap like Ivory, then rinse and dry thoroughly.
- If necessary, shave the area, keep it oil-free, and avoid lotions or ointments before applying the wafer.

3. Handling the Eakin Seal:
- It's okay to leave small, well-adhered remnants of an Eakin Seal on the skin and place the new seal over them.
- Wet your fingers before handling the seal to prevent it from sticking to your gloves or fingers.

4. Removing stubborn residue:
- Use warm, wet, soapy cloths to gently wipe the area; warm water helps dissolve the hydrocolloid.
- If possible, let warm water run over the area in the shower to speed up the process.

5. Protecting the skin:
- Dust irritated areas with a protective stoma powder like Orahesive Powder before applying the seal.

6. Application tips to prevent leakage:
- Cut the ring opening slightly smaller than the stoma so the seal fits snugly.
- Warm the wafer and seal with a hair dryer before and after placement while pressing firmly to improve adhesion.
- Ensure the patient is positioned to minimize creases; extra attention or convexity may be needed for those with obesity or skin folds.

7. Troubleshooting persistent leaks:
- Evaluate the stoma shape and body contours, as product choice often involves trial and error.
- Reach out to the manufacturer’s stoma-care nurse for additional guidance specific to the products being used.
