Healing a skin tear under colostomy wafer - Need advice!

This topic is about dealing with a skin tear that occurred beneath a colostomy wafer. The person who experienced this is looking for ways to help the wound heal faster and to prevent leaks from their ostomy appliance. Here are some practical suggestions and insights that might help:

1. Re-position or adjust the appliance:
- Try moving the wafer opening or using a different system so that the adhesive doesn't sit directly on the tear.
- If the tear is near the edge of the adhesive, consider cutting a window or hole in the wafer over the tear to allow the wound to breathe.

2. Isolate the wound with a protective interface:
- Place a strip of 3M Micropore tape over the tear, then put the wafer on top. Change the tape only when the appliance needs changing to keep it undisturbed.
- Apply a small mound of ostomy powder directly on the tear without spreading it. This prevents the wafer from sticking to that spot while allowing the rest to seal normally.

3. Switch to skin-friendly or healing-promoting wafers:
- Use wafers that contain ceramide, like those from the Hollister CeraPlus line or Coloplast SenSura Mio with Brava Protective Ring, to help with moisture balance and skin repair.

4. Consider alternative dressings:
- A thin hydrocolloid dressing, such as DuoDerm, or a specialty product from Nu-Hope can protect the area under the wafer until it heals.
- Continue using alginate only if you need to absorb exudate; otherwise, leave the wound more open to air.

5. Promote air exposure when possible:
- Allow brief periods without the appliance, or use the cut-out window method mentioned above, to let moisture evaporate and speed up healing.

6. Seek professional guidance:
- Consult an ostomy nurse either in person or through telehealth. Nu-Hope offers telephone consultations with certified ostomy nurses.

7. Investigate underlying causes:
- Consider whether factors like topical steroids, fragile skin, or the technique used to remove the adhesive contributed to the tear, to help prevent it from happening again.
