Appliance Changes Without Barrier Ring or Spray: Seeking Advice

This topic is about managing severe irritation around the stoma, known as peristomal irritation, and dealing with frequent appliance blow-outs. The person sharing their experience had success with Coloplast Brava rings but faced issues after switching to Eakin seals. They tried going without a barrier spray or ring, using only a Sensura Mio flange, and surprisingly, it lasted for 1-2 days even on irritated skin. However, after consulting a stoma nurse and returning to using a spray and ring, the wear time dropped significantly, leading to a near blow-out.

Here are some helpful insights and advice shared by others:

1. **Going Barrier-Less:**
- Some people find success wearing just the wafer/flange without a ring. Key steps include cutting the opening to fit the stoma exactly, warming the wafer before application, and using skin-prep only if it doesn’t reduce adhesion.

2. **Protecting Raw Skin:**
- Consider using SkinTac wipes or 3M Cavilon Advanced Skin Protectant for raw skin without reducing stickiness.
- For treating rashes and open spots, try "crusting" with stoma powder and skin-prep wipes, Benadryl spray followed by powder and barrier spray, Flonase on raw spots, or Vicks VapoRub for short-term pain relief.

3. **Barrier Strips and Flange Extenders:**
- Use C-shaped Coloplast or Hollister strips, or Micropore tape, directly on intact skin around the irritated area before placing the wafer on top. This can protect broken skin and extend wear time.

4. **Managing Rings:**
- If hydrocolloid rings swell and lift the wafer, try placing the ring on top of the wafer around the stoma. Salts Aloe rings are praised for healing but are not available in the U.S.

5. **Product Combinations:**
- Some combinations that have worked for others include Coloplast Sensura Mio Flex wafer with Marlen Ultraseal ring on top, Coloplast 1-pc convex drainable with SkinTac wipe, and Convatec moldable 2-pc with Convatec skin-barrier spray only.

6. **Controlling High-Output Ileostomies:**
- Loperamide (Imodium) can slow motility and is not harmful to kidneys, though it may cause urinary retention. Bulk-formers like Metamucil can absorb water and be alternated with loperamide. Diet tweaks such as bananas, pasta, marshmallows, and oral rehydration drinks can also help.

7. **Dealing with Supply Shortages:**
- Keep an emergency stash and consider reaching out to fellow members for extra supplies. Low-cost generic rings and wafers can be ordered from AliExpress if time allows.

These insights aim to help manage peristomal irritation and improve the wear time of ostomy appliances, providing some relief and comfort.
