Stoma Placement Concerns for Upcoming Urostomy Surgery

Choosing the best spot for a stoma before a urostomy can be a bit of a puzzle, especially when you want to make sure it fits well with your lifestyle and medical needs. The person in this situation is preparing for their urostomy on August 19 and is curious about the placement of their stoma. Their surgeon has suggested a spot about 3 inches above and to the side of the belly button. However, they’ve noticed that many stomas are placed level with or below the navel, which seems more discreet and allows for a support band to sit comfortably at the waist. They’re wondering if a higher placement might be better for medical reasons, like preventing the pouch from kinking when sitting, and are looking for advice from others before discussing it with their surgeon.

Here are some helpful insights and advice from others who have been through similar experiences:

1. The most important thing is to have a flat, smooth surface for the stoma so that the wafer seals well. The contours of your skin are more important than the height of the stoma.

2. Some people have had challenges with very high stoma placements. For instance, one person with a high stoma has to change their appliances twice a day and is considering revision surgery. They suggest involving an ostomy nurse in the site selection process, not just the surgeon.

3. On the other hand, some find a higher placement beneficial. One person had their ileostomy moved higher after experiencing leaks and found the new position easier to conceal under a shirt and more functional.

4. Your body size and shape can influence the best placement:
- Shorter individuals or those with softer bellies often prefer a site a little below or beside the navel for easier coverage by clothing or wraps.
- Taller or heavier individuals might have stomas placed higher, but this can make it harder to hide the pouch.

5. Practical tips include trying to place the stoma so that waistbands won’t rub against it, ensuring you can see it easily when changing, and making sure it stays clear of skin folds when you sit or bend.

6. Many people stress the importance of involving a WOC (wound-ostomy-continence) nurse in the final site marking to balance both medical and lifestyle needs.
