Need Help with Round Sore Above Stoma

This topic is about dealing with a persistent round sore that has developed just above the stoma, near the edge of the wafer. The person experiencing this issue has tried various treatments, but the sore continues to worsen, even oozing and eroding into the second skin layer. They are seeking advice on how to heal and protect the sore effectively. Here are some suggestions and insights shared by others:

1. Have the sore clinically assessed. It's important to rule out any underlying issues like a fistula. Consulting with both an ostomy nurse and a surgeon can help prevent the sore from getting worse.

2. Consider cauterization. One person found that using silver-nitrate cauterization, initially done by a doctor and then self-performed, helped slow the sore's enlargement and kept it stable.

3. Try antimicrobial silver gel. A silver-based wound gel, such as SilvaSorbGel, was effective for someone else, clearing a similar sore within four days. A small amount was applied directly under the adhesive, even though it slightly reduced the wafer's stickiness.

4. Explore ozone therapy. A caregiver used a tabletop ozone generator to deliver ozone to the sore, which led to rapid healing. The method involves covering the stoma with a moistened cap, directing the ozone tube at the sore, trapping the gas with an inverted plastic bowl, and insulating with disposable bed pads and towels. Afterward, powder the area and reapply the wafer. This treatment is repeated whenever redness appears, and the patient takes Imodium and stops intake a few hours before appliance removal to minimize output during treatment.

5. Use protective hydrocolloid. Cutting a piece of ConvaTec DuoDERM slightly larger than the sore and placing it over the area can help isolate and protect it. The ostomy appliance is then applied on top.

6. Consider antifungal treatment. Sometimes, a fungal infection might be the cause. Applying a prescribed antifungal stoma powder can quickly resolve the issue once confirmed.
