Stoma Size - Importance of Pouch Opening?

This topic is about finding the right fit for your stoma pouch or wafer opening. It's important to ensure that the opening matches your stoma size to protect your skin and improve the wear time of your pouch. Here are some helpful tips and insights:

1. Opening Size Basics
- The hole in your wafer or pouch should closely match your stoma's diameter, leaving no bare skin visible. A snug fit, even as small as a 1/32-inch gap, is better than a larger 1/8-inch gap.
- If your stoma size falls between two commercial sizes, consider using cut-to-fit products and trim them precisely to your needs.

2. Using Barrier Rings/Seals
- Moldable barrier rings, like Eakin Moldable Barrier Rings, can help close any gaps, melt to the stoma, and protect your skin when the wafer is slightly too large.
- Rings are often tidier and more effective than paste, providing a softer, tighter seal.
- Combining a ring with a properly centered wafer can extend wear time to 2-3 days, even with liquid output.

3. Sampling and Sizing Tools
- Request free samples from suppliers in three sizes: your current size, 1/8 inch smaller, and 1/4 inch smaller.
- Ask for both convex and flat wafers, along with barrier ring samples.
- Use the manufacturer’s stoma measuring card to quickly find the exact size.

4. Convexity and Product Combinations
- Convex wafers can prevent thin output from undermining the adhesive, especially for flush or irregular stomas.
- Some users have found that pairing an Eakin Moldable Barrier with a Coloplast 2-piece drainable bag extends wear time, even if convex one-piece bags leak quickly.

5. Practical Tips for High-Liquid (Ileostomy) Output
- Ensure no skin is exposed, as liquid output can quickly undermine the flange.
- Warm the adhesive ring or wafer before applying it for better adhesion.
- Empty pouches frequently and consider using gas-reducing aids like Beano or Gas-X if gas is excessive.

6. Cut-to-Fit vs. Pre-Cut
- Cut-to-fit options provide the most accurate seal when your stoma size fluctuates.
- Pre-cut options are convenient once your stoma size is stable. If in doubt, choose the smaller size and stretch it slightly rather than using a hole that is too large.
