Preventing Skin Ulcers from Leaks - Share Your Tips!

The topic is about preventing peristomal skin ulcers after a stoma has been relocated. The person sharing their experience had no issues with ulcers until their stoma was moved from just above the belt line to the upper abdomen. Since then, they have experienced several ulcers due to leaks. They have found a routine that seems to work, but it took a lot of trial and error. They are seeking additional strategies to prevent future ulcers.

Here are some helpful tips and insights:

1. Consult a professional: Work with a Wound, Ostomy & Continence Nurse (WOCN) to ensure that the appliance style and adhesive are suitable for your stoma's new location and your skin type.

2. Consider changing appliances: If necessary, try a different appliance. For example, a flat Marlen model that can be cut to fit the ulcerated area has helped some users achieve better wear time.

3. Improve adhesion: Some people have found success by applying an extra-thick layer of adhesive to the wafer before putting it on.

4. Use barrier rings or seals: Many have found that using a barrier ring, like an Eakin seal, can effectively stop leaks and prevent skin breakdown.

5. Try powders for healing and protection:
- Use a thin layer of adhesive powder mixed with niacin powder to help dry and heal active ulcers.
- Nystatin powder can be effective for treating yeast-related rashes, especially in hot and humid weather.

6. Manage uneven skin: Fill in any scar indentations or dips with small pieces of stick barrier material to create a flat surface for better adhesion.

7. Address output and wear-time factors: Medications like Lomotil and fiber supplements such as Benefiber can help thicken output, reducing the chance of leakage under the wafer.

8. Explore effective appliance combinations:
- A 2-piece Coloplast system with a convex baseplate and an Eakin seal can work well for a flush stoma.
- An older Marlen 2-piece reusable faceplate and pouch may offer longer wear time if the fit is correct.

9. Follow belt use guidelines: Avoid tightening the belt too much. Some people wear the belt only temporarily after each change or remove it during the day to allow the skin to breathe and reduce pressure points.
