Irrigation Issues: Intestine Closing Up, Any Advice?

1. Consider muscle (peristalsis) blockage:
- Sometimes, the internal muscle of the stoma can shift over the opening, blocking the cone and even trapping gas.
- One approach is to gently dilate the stoma by inserting a lubricated finger and applying steady pressure until the muscle relaxes. Then, insert the irrigation cone while maintaining firm pressure to prevent it from being pushed out too soon.
- This technique can also be used when inserting stoma plugs to help the muscle gradually learn to stay open.

2. Try a DIY device to "train" the muscle and vent gas:
- A plastic nasal inhaler case, with its outer and inner parts screwed together, can be used as a finger-sized tool.
- Drill multiple smooth, countersunk holes in the tip and sides to allow gas to escape, as a single end hole might get sealed by the muscle.
- Thread two wires through the outer end to attach an adjustable waistband, which helps keep the device in place long enough for the muscle to relax.
- This method has been successful for some, but it requires personal adjustments and should be approached with caution.

3. Address hernia-related narrowing:
- A parastomal hernia can sometimes kink or compress the bowel, blocking water flow.
- Gently supporting or pushing the hernia inward by hand until the initial water flow starts can help. Once the flow begins, irrigation usually proceeds normally.

4. General cautions:
- Always seek guidance from a medical professional or ostomy nurse before trying digital dilation or homemade devices, as what works for one person may not be safe for another.
- Scar tissue from previous hernia surgery can also cause stiffening or narrowing, so a professional assessment is important.
