Alternative Methods for Healing Irritated Skin Below Stoma

I just had a stoma revision to treat a hernia/prolapsed stoma. Now, my stoma is down in a funnel, below the skin. Anyone have a similar situation? What is a simple method to heal the badly irritated skin down in the funnel that immediately surrounds my stoma? It is raw from the 7:00 to 5:00 position, bleeding, and won't heal. Crusting is not working for me.

The most efficient solution is to go back to the surgeon who has created the problem - unless one is grossly overweight, that is.

Maybe others may help you with a less radical suggestion - but for a lasting solution, the stoma is required to have an ability to deposit its effluent clear of your abdominal skin!

Sorry to be direct - but to delay a permanent solution - unless your BMI is high - would be to skirt around the problem.

Weight management is important; to manage a stoma successfully, one needs to achieve a 'flat' surface for the stoma to protrude from in order to clear the interface of your collecting appliance.

There is a lot of dietary advice to be found on the net for a self-help regime if this is necessary.

PS ...... I believe there is an actual 'funnel' kind of collar that some use to act as a channel where there is an inverted stoma below the abdominal skin - I seem to recall this being covered within a relatively recent posting - try using the AI search criteria on this site - it will turn it up for you - BW - J.

G-Day BH,

You have to get that fixed. Please get back to your surgeon ASAP and get it extended to at least 3/4 to 1 inch above your skin.

From prolapse to retracted. You need to find out from the surgeon why your stoma was revised like this.

Yep.. a surgeon should know better. A stoma should be 3/4 to 1 inch above skin level at its smallest shape. Stomas are always on the move due to peristalsis. Yours needs to be fixed, unless there is a physical reason it can't be.

The skin will not heal as long as effluent continues to leak on it. I recommend you try the following from Coloplast:

  1. Sensura Mio Convex Deep Click Barrier
  2. SenSura Mio Click Closed or Drainable Pouch
  3. Brava Elastic Barrier Strips

Perhaps this will press down enough on your abdomen to allow your life investment (stoma) to protrude above the abdomen.

To help the skin heal, I recommend Rx 2% lidocaine gel:

• First, apply lidocaine 2% gel to the desired treatment area during a pouch change, allow it to sit for 5 minutes to facilitate anesthesia, then wipe it off.
• The gel can be removed easily without greasy residue, so it does not interfere with appliance adhesion.
• The skin anesthesia permits thorough cleaning of the site with adhesive remover, followed by a warm water rinse.
• The skin is then dried with a low-temperature hair dryer.

The lidocaine's anti-inflammatory effect decreases skin weepiness — enabling improved pouch adherence.
