Struggling with Ostomy Skin Issues - Need Advice

This topic is about dealing with persistent skin issues and appliance problems after a stoma relocation for someone with a long history of Crohn’s disease. The person had a successful ileostomy on the right side, but after moving the stoma to the left side, they have been experiencing continuous skin rashes and frequent leaks. Here are some insights and advice shared by others who have faced similar challenges:

- The person has tried various products and methods to manage the rash and leaks, including skin-prep wands, barrier pastes, creams, and strict cleaning routines. Despite these efforts, the issues persist.

- They suspect that high liquid output and missed meals might be contributing to the problem. Eating more solid foods seems to help ease the rash.

- They are considering trying new products and methods, such as poison-ivy wipes, aloe flange extenders, Metamucil, and possibly consulting an allergist.

- Some practical tips include using Domeboro aluminum-acetate soaks and apple-cider-vinegar soaks to help with the rash, followed by drying and applying a light dusting of ostomy powder before placing the wafer.

- Aloe-infused flange extenders or barrier rings can be used to cover the rash and promote healing. Samples can be requested from suppliers.

- To protect broken skin, inserting a thin flange extender between the skin and wafer can help keep adhesive off sensitive areas.

- Switching to Dove Sensitive Skin soap or Dial Antibacterial soap instead of Ivory soap might be beneficial. Some people find that cleansing with warm water only works best for them.

- Thoroughly drying the skin with a hair dryer on a low setting and pressing the wafer by hand for a couple of minutes can improve the seal.

- Using a combination of powder and barrier spray may provide better adhesion than powder and wipes. Apply a very light dusting of powder only on broken skin.

- Extended-wear convex systems with barrier rings are suggested for stomas that are flush or in a valley.

- To thicken the output and reduce skin damage, consider using Metamucil powder, eating marshmallows before changing the appliance, or trying Imodium and probiotics.

- For suspected yeast infections, Nystatin prescription powder or over-the-counter athlete’s-foot spray applied on gauze around the stoma has helped some people.

- If an allergy is suspected, consulting an allergist might be helpful. Flonase spray lightly applied to dry peristomal skin has been beneficial for some.

- Moisture under the wafer can shorten wear-time, so if you hear a "squish" sound while pressing, it might be time to change the appliance sooner.

- Taking no-pouch showers and allowing the stoma to air-dry for about an hour twice a week can help maintain healthy skin.

- It's important to remember that every rash can have a different cause, so trying single changes methodically and consulting with an experienced ostomy nurse can be valuable.
