Dealing with Stoma Pancaking: Tips and Experiences

This topic is about dealing with a retracted stoma and the issue of "pancaking," where the output sticks to the stoma or wafer instead of falling to the bottom of the pouch. 1. Consider surgical revision:
- Some people suggest that a retracted stoma can cause ongoing issues. Ideally, the stoma should protrude about ¾ inch. A surgical revision to extend the stoma might help reduce pancaking and appliance failures.

2. Use interim mechanical aids:
- Try using convex wafers and an ostomy support belt to help with the retraction. However, be mindful that these can sometimes cause skin pressure issues over time.

3. Keep air in the pouch:
- After emptying the pouch, gently blow or pump a little air into it and push the air toward the stoma. This helps create space for the output to drop. If your pouch has a charcoal filter, temporarily cover it to keep the air inside.

4. Warm-water pouch rinse (for drainable systems):
- Open the bottom of the pouch, add about ¾ cup of warm water, close it, and swish to loosen the "pancake," then drain. Some users have done this for years without leaks, as long as the water stays below the wafer line.

5. Two-piece removal and clean:
- If you use a two-piece system, you can remove the pouch, clean the stoma and wafer face if needed, and reattach it before any leaks develop.

6. Lubricate the pouch:
- Use a commercial pouch lubricant or a few drops of cooking oil inside the bag to help the output slide down more easily.

7. Adjust fiber and diet:
- Adding Benefiber, oatmeal, or other fiber can bulk up the output, but be prepared for an increase in volume. Make sure to drink enough water, especially if you consume a lot of milk, as it can thicken the output and worsen pancaking. If you enjoy milk, try to balance it with adequate hydration.

8. Consider specific products:
