Excess Gas Relief - Need Suggestions for Ileostomy Reversal

This topic is about finding ways to manage excess gas for someone who has been living with an ileostomy for over a year. Despite being careful with their diet, they still experience a lot of gas, especially at night, which inflates their pouch and causes concern about potential leaks. With an ileostomy reversal coming up, they are seeking advice on how to handle this issue. Here are some suggestions and insights shared by others:

1. Dietary Changes
- Reducing carbohydrates, particularly those from wheat-based grains, can significantly decrease gas production.
- Consider getting tested for celiac disease, as undiagnosed celiac or gluten intolerance might be causing the gas issues. If the test is negative, try eliminating wheat and gluten from your diet step by step.

2. Appliance Venting Products
- The Osto-EZ-Vent is a product designed to help release gas from the pouch. Some people find it useful, though opinions on its cost and effectiveness vary.

3. Manual Gas Release ("Burping")
- Learn to "burp" your two-piece pouch by briefly separating the coupling while lying down to let the gas escape. This can be done during the night.
- When not at home, some prefer using a top-mounted vent or the Osto-EZ-Vent instead of opening the coupling.

4. Switch Pouch Size at Night
- Consider using a larger pouch at night to accommodate more gas and output, which can help reduce the risk of leaks. This is most effective with a two-piece system.

5. General Management
- Managing gas is crucial before the reversal surgery, as distinguishing between gas and stool will be important afterward.
- It's a good idea to consult with your GI team early to address any dietary intolerances or underlying conditions before the surgery.
