Wafer fit around stoma - snug or loose?

This topic is about finding the right fit for the wafer opening around a shrinking ileostomy. The person is three weeks post-surgery and is having trouble cutting the wafer opening correctly. The home-health nurse suggests making the hole larger to avoid injuring the stoma, but this has led to skin issues due to effluent pooling. Here are some helpful insights and advice:

1. **Sizing & Cutting**
- It's generally recommended to have an opening about 1/8 inch larger than the stoma to allow for expansion and prevent sharp edges from causing injury.
- For irregularly shaped stomas, you can trace two half-circles to create an oval guide or use templates from companies like Nu-Hope Laboratories.
- Once the stoma size stabilizes, consider switching to pre-cut wafers to avoid cutting errors and improve wear time.

2. **Filling the Gap / Sealing**
- Protect any exposed skin with moldable barrier rings or stomahesive paste, which can be stretched to fit snugly around the stoma.
- Convex wafers and moldable technology are often praised for preventing leaks and eliminating the need for cutting.
- For high-output ileostomies, using convex wafers with barrier rings can improve the seal and pull the stoma forward.

3. **Skin Protection & Healing**
- Use the "crusting" method by dusting irritated skin with ostomy powder, blotting off excess, and sealing with a non-sting barrier wipe or spray.
- Soothing creams like zinc and castor-oil cream or Neosporin can help heal raw areas if allowed to dry before applying the appliance.
- Alternate the orientation of the wafer with each change to give different skin areas a rest.
- If itching occurs, consider changing the appliance more frequently until the skin heals.

4. **Adhesion Tricks**
- Warm the wafer and skin with a hair dryer on low or by cupping your hands to improve adhesion.
- Use a PVC pipe cap or similar object to apply even pressure over the wafer.
