Help! Washing stoma without removing adhesive?

This topic is about managing leaks and cleaning the stoma area without losing the adhesive seal, which can be a challenging experience for those with a colostomy. The person sharing their story is dealing with frequent leaks, skin irritation, and emotional distress. They are seeking advice on how to maintain the adhesive seal while showering and are curious if others face similar issues. Here are some helpful tips and insights:

1. Appliance Wear-Time and Product Trials
- Many people with ostomies can wear their appliances for 4 to 7 days, and some even manage 10 to 21 days using convex moldable wafers.
- Contact manufacturers like Hollister, Coloplast, and ConvaTec for free samples to find products that suit your skin.

2. Preparing the Skin Before Applying the Wafer
- Use an adhesive-remover wipe or spray to completely remove old adhesive.
- Avoid soaps with oils, creams, or perfumes. Opt for plain yellow Dial soap, hospital skin-cleansing cloths, or just warm water.
- Ensure the skin is completely dry; some use a hairdryer on a cool or warm setting.
- Optionally, use a skin-prep or protective wipe to protect the skin.
- Lightly dust irritated skin with stomahesive or stoma powder and remove any excess.

3. Building a Secure Seal
- Cut or shape the wafer opening to fit snugly around the stoma to prevent leaks.
- Consider products known for reliable adhesion, such as Hollister New Image system, Coloplast “Click” system, and Eakin Cohesive Seals.
- Warm the wafer after application by holding your hand over it for 60 to 90 seconds to activate the adhesive.
- Some people use additional security tape or an ostomy belt for extra support.

4. Showering and Washing
- Most people shower with the appliance on, covering the filter sticker or using a sandwich bag to keep it dry.
- Let plain water run over the stoma without scrubbing. Some use mild, residue-free soap if rinsed well.
- If showering without the pouch, be ready to dry, prep, and apply a new wafer immediately after.
- After showering, pat the pouch dry or use a hairdryer to dry the fabric backing.

5. Cleaning the Pouch and Reducing Pancaking
- While seated on the toilet or in the shower, squirt warm water (with a drop of baby oil if desired) into the drainable bag, swish, and empty to keep the area clean.
- Place a small piece of toilet tissue in the tail or a smear of Vaseline inside near the outlet to help output slide down.

6. Dietary and Output-Control Tips
- Favor low-fiber, higher-protein foods and avoid carbonated drinks and excessive roughage to manage output and gas.
- Colostomates might consider irrigation to reduce daily output and leaks, with guidance from an ostomy nurse.
- Eating 4 to 5 marshmallows 15 to 20 minutes before a change can temporarily slow output.

7. Skin Irritation and Burning
- Use a no-sting barrier film before applying the wafer to protect broken skin.
- Breaking the cycle of leaks and skin damage can improve adhesion and reduce leaks once the skin heals.

8. Emotional Support
- Leaks and frequent changes are common initially, but persistence and the right products can resolve these issues.
- Feelings of depression and fear are normal; seek support from stoma nurses, local support groups, and educational resources like Phoenix Magazine and online videos.
