Finding the right pair of jeans when you have an ileostomy can be a bit of a challenge, especially if you want to avoid the waistband pressing uncomfortably against your pouch. Here are some friendly tips and ideas to help you find jeans or trousers that fit well without needing to go up a size:

- Consider stretch or flex-waist styles:
- Look into Duluth Trading Post "Flex" pants or jeans, which offer a bit of stretch around the stoma area.
- Public Rec jeans have the look of Levi’s but feel like sweatpants, with a flexible waistband.
- Many brands now offer jeans with Lycra or spandex; try any "stretch" line for added comfort.

- Opt for a higher rise:
- A higher rise can position the waistband above the appliance. For instance, Levi’s 505 has a higher rise than the 511.
- High-waist cuts in women’s or unisex styles can also be beneficial.

- Try a different style altogether:
- 32 Degrees high-waisted elastic pants are comfortable, though the inseam is around 34 inches.
- Match Men’s "Wild Cargo Pants" from Amazon have a higher waist and a roomy fit.
- Cargo or elastic-waist trousers are a great alternative, especially in warmer weather.

- Add stretch to your existing jeans:
- A tailor can modify the waistband by inserting elastic or an extender that hides under a belt.
- For a DIY approach, cut a small piece of the waistband and sew in elastic, which works well on old favorite jeans.

- Protect the stoma to wear tighter clothes:
- A "StomaDome" guard can fit over the appliance, protecting it from belts and tight waistbands, allowing you to wear slimmer jeans and sports gear.

- Manage the pouch differently with jeans that fit in the legs:
- Roll a 10-inch drainable pouch upward and tuck it under the underwear elastic; this can make wearing skinny jeans more comfortable.
- Experiment with the pouch orientation, whether vertical or horizontal, to see which works best with the waistband.

- If a larger waist results in baggy legs, consider custom alterations:
- A local tailor can adjust the seat and legs after upsizing the waist or insert small elastic insets where needed.

- General reminders:
- Look for men’s jeans labeled "flex," "athletic," or "comfort stretch."
- Loose-thigh cuts provide extra room for pouch expansion, even if the waistband is snug.
