Irritation at Bottom of Stoma: Anyone Used Mupirocin Ointment?

This topic is about dealing with persistent redness and irritation on the skin right beneath a stoma. A person experiencing this issue has been prescribed Mupirocin Ointment, a topical antibiotic, by their wound-care physician. They are curious to know if others have used this ointment and how effective it was for them.

Here are some helpful insights and advice:

1. Mupirocin is an antibiotic ointment typically used when a bacterial infection is suspected, rather than a fungal rash.

2. If a fungal cause is suspected or if you are waiting to see a doctor:
- Consider using an over-the-counter antifungal like Desenex powder. Apply it and then "crust" it with barrier wipes or film to ensure the pouching system adheres properly.
- Nystatin powder is another option that can be applied and crusted in the same way.

3. For damaged peristomal skin, you can try the crusting technique:
- Dab the area with a 3M Cavilon swab (skin prep).
- Sprinkle stoma powder on the moist area.
- Re-dab with the swab and repeat the powder and dab process two more times.
- Finish with a spray of 3M Cavilon barrier film over the entire area.
- Add a moldable ring, apply the wafer, and attach the bag. Many people report 7–15 days of wear time with this method.

4. Redness under the stoma can also be caused by mechanical irritation from sitting. Reducing pressure and protecting the skin with crusting or barrier rings can often help.

5. Make sure the appliance template fits snugly so no skin is exposed. If there is a small gap, you can puff a bit of stoma powder into it before applying the wafer.

6. After applying the pouching system, press and hold the wafer area for about 30 seconds to strengthen the seal.

7. Using a stoma support belt or wrap can help reduce appliance drag during activities. Some examples include generic wraps available on Amazon and products from Stealth Belt.
