Day 12 Post-Op: Managing Leaks and Stitches

This topic is about the experiences and challenges faced by someone who is twelve days post-ileostomy surgery. They are dealing with issues like leaks, skin soreness, and managing their output. Here are some helpful tips and insights shared by others who have been through similar situations:

- The district nurse removed the skin clips, which caused only a mild sting.
- The skin around the stoma is still very sore, especially after stopping the use of cleansing wipes just a day ago.
- There have been two unexpected leaks, one minor and one with a full pouch, even though the routine hasn't changed.
- Imodium tablets, prescribed to thicken the output, haven't been effective. There's a concern about liquid output and ballooning during an upcoming flight.
- A visit from the stoma nurse is scheduled for further guidance.
- Despite feeling exhausted and a bit grumpy, there's an effort to stay positive by shopping for a stylish carry-bag for supplies.

Here are some pieces of advice and insights that might help:

1. Thicken Output
- Consider switching from Imodium tablets to liquid form, and consult with a healthcare professional about possibly using higher doses than over-the-counter recommendations.
- Combining Imodium with Codeine Phosphate might help slow and thicken the output, but professional advice on dosing is essential.
- Ileo Gel sachets can be placed inside the pouch to absorb liquid output, which some find more effective than Imodium alone.
- Eating plain crackers and pretzels can help firm up stool. Keeping a food journal can also help identify foods that cause watery output or gas.

2. Pouching Systems & Accessories
- Many start with a large one-piece drainable pouch post-op, like those from Salts, and transition to smaller two-piece systems as output settles.
- The Convatec Natura Moldable Dermahesive wafer forms a seal that moves with the body, helping to minimize leaks.
- Coloplast SenSura and Latina one-piece pouches are favored by some for their comfort and security.
- Placing a bath towel under you at night can provide peace of mind while leaks are still common.

3. Skin Protection & Soreness
- Use adhesive protective powder around the stoma to dry moist skin, promote healing, and improve wafer adhesion.
- NO-sting 3M Cavilon barrier wipes can protect irritated skin and have helped others heal quickly.

4. General Encouragement
- It's normal to experience leaks, liquid output, and skin soreness in the first weeks. Most issues improve with time and product adjustments.
- Rest is crucial this soon after surgery. Many patients are still in the hospital at day 12, so be gentle with yourself.
